– On the screen that will open, "Sorry for your decision to leave. Disabling your account will not delete your user information. Contact us at support@twitch.tv and let us know if there is anything we can do to change your decision." after reading the information, you can optionally indicate your reason and click the "Disable My Account" button.

Permanent Deletion of Twitch Account

– "Are you sure you want to delete your Twitch account? This will permanently delete all of your content and personal information, and you will no longer be able to request the user name as your user ID." After reading the warning you can optionally indicate the reason and click the "Delete Account" button.
